Information collected
The form collects work email, role, organization, Jira Service Management responsibility, the request process to be protected, process importance, experience or concern about silent JSM failures, self-service setup preference, written-feedback agreement, and optional JSM scale, willingness-to-pay range and notes.
Do not submit Jira credentials, API tokens, Atlassian account IDs, customer or employee personal data, sensitive request content, or confidential Jira configuration exports.
How information is used
Application information is used to evaluate and manage Request Sentinel Early Access, provide written onboarding where an application is accepted, understand product fit, and collect feedback and willingness-to-pay research.
The implementation uses Cloudflare Pages for hosting, a Cloudflare Pages Function for form handling, Cloudflare D1 for application storage, and Cloudflare Turnstile for bot protection. The website has no analytics or tracking scripts. Application data is not sold.
Data minimization
The application pipeline does not intentionally store IP addresses, User-Agent strings, referrers, browser fingerprints, Turnstile tokens, Turnstile verification responses, Jira credentials, Atlassian account IDs, API tokens or raw request JSON.
Retention
Rejected or withdrawn applications are deleted after 90 days. Accepted/current Early Access applications are retained while the Early Access relationship is active. Inactive or completed applications are reviewed for deletion after 12 months.
Withdrawal and deletion
You may request withdrawal or deletion by emailing [email protected]. Deleting an Early Access application does not delete any Jira issues that may later have been created by the Request Sentinel product.
Product data
The Request Sentinel Forge product uses Forge-hosted compute and storage and does not use an external business-data backend in the current architecture. Product-data handling is separate from this Early Access application notice.
